NEED TO KNOW: A proxy signature is ONLY permitted if the individual is physical unable to sign the documents, and must be requested by the individual. Underneath the proxy signature you should make a note stating that you the notary signed on behalf of the individual. Also be sure to ID both the disabled individual and the witness.
