As a member of the multi-disciplinary Dementia Outreach Team within Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will undertake specialist assessments of care home residents living with dementia.

Applicants must demonstrate strong Occupational Therapy skills and experience supporting people with dementia, ideally within community settings. Experience in contracture management and effective caseload management is essential.

Join CityCare as an Occupational Therapist.

We are seeking a skilled, enthusiastic and motivated Occupational Therapist with an interest in holistic practice to join the Integrated Reablement Team, including the Urgent Community Response Service across Nottingham City.

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and Reablement programmes, as well as complex discharge planning experience.
