SUMMARY DESCRIPTION: Coastal Community Action Program is looking to hire a Case Manager for our Olympia office. This position provides advocacy, assessment, case management, information, referral and outreach for eligible program participants. Works closely with the primary health care providers, case managers, state, and local agencies who provide supportive services for HIV positive individuals. This position demands a high level of cultural competency of the unique needs of the LGBTQIA++ community, communities of color, and other vulnerable populations. This position is a full-time position working from either our Olympia office or our Centralia office, depending on location of the preferred candidate, serving clients in Thurston, Mason, and Lewis Counties.

Job Posting: Landing Pad Care Coordinator II - Float
SUMMARY DESCRIPTION: The Landing Pad Care Coordinator II (Float) performs all of the same direct-service duties as the Landing Pad Care Coordinator II and is additionally scheduled to provide first-call coverage for shift callouts. The Float Care Coordinator is assigned a primary shift and works four scheduled days per week. On their three off-schedule days, the Float Care Coordinator is not on-call and is not required to be available. They serve as the first contact when the supervisor is seeking coverage for an unplanned absence. Shifts accepted on available days are paid at the standard hourly rate and will not exceed 40 paid hours per week without supervisor authorization.
This role is well suited for someone who values schedule flexibility, is comfortable working across multiple shift contexts, and thrives in a role that requires adaptability and broad program knowledge. The Float Care Coordinator plays a critical role in ensuring the Landing Pad meets its contractual two-person minimum staffing requirement at all times.
This is a full-time, hourly position located at the Center for Community Health in Clark County, WA.

The HCS Administrative Support II provides full-time administrative support to Care Coordinators, Program Specialists, and leadership across CCAP’s Housing & Community Services programs in Grays Harbor, Pacific, and Lewis Counties. This position is based at the Aberdeen office and manages a broad range of participant-focused administrative tasks that reduce documentation burden on direct service staff, ensure program compliance, and support continuity of participant services.
Core functions include recertifications, reauthorizations, benefits administration, FCS application support, assignment tracking, HEN subsidy coordination, and logistical participant support. This position does not carry a direct client caseload but works in close coordination with Care Coordinators and program staff to support participant outcomes.
This is a full-time position based in Aberdeen, Washington.

SUMMARY DESCRIPTION: Coastal Community Action is looking for a Supported Employment Specialist/Job Coach for our Centralia office. This position is responsible for all aspects of working with people with developmental disabilities in a supported employment program and/or Community Inclusion. This position is responsible for coaching participants at their work/volunteer site, and assisting them in addressing barriers to employment. This position is also responsible for developing potential job opportunities within the community in for-profit agencies and businesses and for providing technical assistance to employers in addressing barriers to employment. Community Inclusion services focuses on working with people with developmental disabilities on activities that are typically experienced by the general public while ensuring health and safety, promoting positive image and relationships in the community. Staff will be expected to be working with a client or on behalf of a client 80% of the day.
